I take it you mean, no matter which icon you choose in:
Winamp Prefs (Ctrl+P) -> FileTypes section -> File icon - slider.
You've not being changing the icon selection in Prefs -> Display instead? eg. the one for taskbar/systray icon.

If you've definitely been changing the correct setting, but it doesn't appear to be working, try this instead:
Goto: Prefs -> FileTypes; uncheck "Register files on Winamp start"
For the sake of it, choose your preferred icons for media file & playlist file.
Close Prefs & exit Winamp.
Now goto Windows Folder Options (c/o Start -> Settings) -> File Types tab.
Scroll down for "Winamp Media File", click "Edit", click "Change Icon"
Choose your preferred icon from the list within winamp.exe
Click "Apply", then "Okay"
Repeat this for "Winamp Playlist File"

This should fix your problem.

btw, I suppose it's worth checking to make sure that no files in the Winamp dir (including the dir itself) have the Read Only attribute.
Main file to check is winamp.ini
This is the file which stores all the relevant settings.
First of all, if you've used the "customize this folder" tool, undo it!
Next, from within the dir, select all files -> Properties.
Make sure there's no checkmark under "Read Only"

I do something which gives me more freedom.

I disassociated MP3 and WMA from Winamp.

Then I recreated those filetypes in my folder options->filetypes, and gave them the same actions as Winamp Media File has. And of course I gave them their own icons

I even did the same for M3U, although I couldn't change the icon. what I did do to it was add an action that allows me to open my M3U's in windows' Notepad to manually edit.

Here are the icons I use (reply if you want them and I'll post a download link):
MP3
WMA

Funnily enough, that's what I do too
I've got about 10 different actions listed under MP3, M3U, PLS, WAV . . . etc.
. . . in fact, all multimedia filetypes! (audio, video, images, etc)

I kept them associated with Winamp (exept Quicktime !!), but to make effective the new icons, I sometimes have to restart the computer.
It's incredible how Internet Explorer can cache. The worst is when you've got a defective CDROM, that Windows mistakes to be empty : you've got to restart the computer in order to try again.
